St. Charles Health System a non-profit organization has incorporated the new Community Health Department dedicated to improving the health of Central Oregonians. We provide a collective voice on issues concerning health care access, coverage, prevention, care coordination and wellness for Central OregonOur Community Services
SharedCare: A community health plan designed to provide affordable health care benefits to uninsured community members through their employers.
Care Coordination: Health care
services that focus on all health an human service connections for
individuals and families of the tri- county region. Individuals and/or families are partnered with a Community Health Worker to provide support, assistance, health care system navigation, identification and elimination of barriers to health and human service access and community resources at no cost to those in need.
Trails to Health: A prevention and wellness program that empowers employees and their families to make healthy lifestyle choices.
