SharedCare Plan Costs

The SharedCare Plan is modeled after a nationally recognized Multi-Share health coverage Program. Costs for the program are shared among the employer, employee, and the community. The SharedCare plan is an initiative of healthmatters of Central Oregon and is made possible by numerous community partnerships, the passage of Oregon State Senate Bill 862 with oversight by the Oregon Office of Health Policy and Research under the direction of the Oregon Health Authority.  

The monthly program cost:

   Health Improvement Option  Standard Option
 Employer  $54  $75
 Employee  $54  $75


While the employer must contribute at least 50% of the employer/employee premium, they may choose to contribute more. The employer must offer coverage for dependents of eligible employees, but the employer is not obligated to contribute to the dependents' monthly cost. If the employer opts to contribute, they may give any amount or percentage as long as the contribution rate is the same for all employees' dependents. There is no minimum contribution requirement for dependent coverage.

Copays and Out-of-Pocket Expenses

Members must use in-network health care providers in Crook, Deschutes and Jefferson Counties; and be prepared to pay applicable co-pays at the time of service. Click the link to see a list of services and associated co-pays. Medical Services and Co-Pays.pdf
