Health Improvement Option Requirements


The Health Improvement Option is designed to empower members to take control of their health.  The SharedCare Plan was designed with an emphasis on primary care and prevention. We encourage members to seek out medical care and provide them with the support necessary to proactively manage health issues before they become chronic or a crisis. Those choosing to enroll in the Health Improvement option are making a personal investment in their health and will be required to take the actions necessary for health improvement.

The member will work with our Nurse Care Manager to select one or two health goals they wish to work on, such as lose weight, quit smoking, change their diet, or better manage their diabetes. Health Improvement Option members must attend classes and participate in activities related to their health goals. The Nurse Care Manager is readily available to help members achieve their goals through health coaching, providing resources, and recommending classes.

As long as the member is actively working on his/her health goals, the monthly fees and co-payments are at a reduced rate.

Health Improvement Option Members Have the Following Responsibilities:

  • Complete a Health Risk Assessment
  • Select 1 or 2 health goals to work on over the next year
  • Meet with a Nurse Care Manager within three months of signing up for the Health Improvement Option to establish a plan to achieve health goals
  • Attend health activities or health classes as recommended by Nurse Care Manager within 12 months of enrollment. If a member has a persistent or chronic health condition such as diabetes, high blood pressure or obesity, they will need to meet with a Nurse Care Manager and participate in educational classes related to the chronic health condition.
If a member does not meet the conditions and requirements of the Health Improvement Option, they will be transferred to the Standard Option and will no longer receive the lower rates. Members may change plans during open enrollment periods designated by SharedCare but must remain in the selected plan for at least six months.
