SharedCare a service of St. Charles Health System provides the choice of two benefit programs: the Health Improvement Option and the Standard Option.

Health Improvement Option

The Health Improvement Option encourages members to make a personal investment in their health and take the actions necessary for health improvement. This option includes a wide range of health services (such as doctor visits, hospital care, home care, diagnostic tests) AND rewards members for taking steps to a healthier lifestyle with lower fees and co-payments. Members who choose the Health Improvement Option complete a Health Risk Assessment and then meet with a Nurse Care Manager to discuss what they can do to improve their health. Standard Option

The Standard Option does not require the members to complete the requirements related to improving their health; however, Standard Option members are required to complete a Health Risk Assessment upon enrollment. The Standard Option includes the same set of covered services as the Health Improvement Option, however there is a higher monthly cost and higher co-payments than the Health Improvement Option. Those selecting the Standard Option who have a chronic health condition will be required to meet with the Nurse Care Manager and participate in educational classes related to their chronic health condition.
