Welcome to the SharedCare Plan
SharedCare a service of St. Charles Health System is excited to partner with tri-county health care providers so we can offer affordable health coverage to local uninsured, small business employees who may otherwise not have the means available to afford health services.The SharedCare Plan is intended to empower the patient to be an active participant in improving their own health. The Plan focuses on well care instead of sick care by emphasizing preventative medicine. Together we can help establish a healthier community and lower overall health care costs for the future by providing access to primary care providers to detect health problems earlier and develop appropriate treatment plans.
SharedCare is a tightly managed and coordinated approach to health care that seeks to balance effective use of health services and community resources with health education, support for healthier lifestyles, and member responsibility.
