Community Resources and Education

The SharedCare Plan is not modeled like traditional health programs because we want to emphasize health improvement and empowerment. In order for our program to be successful in improving health and maintaining costs, we have developed several community partnerships to assist us in achieving our goals.

We strongly believe in education and offer classes on chronic disease and other health issues. We provide support for members interested in lifestyle changes such as weight reduction and smoking cessation so members can achieve optimal health. Most importantly, we care about your health. We will partner with you to improve your health today so you can have a better quality of life.

For members on the SharedCare Health Improvement Option, your Nurse Care Manager will recommend classes and resources in the community that match your healthcare needs and goals. Community health care providers and other organizations offer a variety of classes, clinics and other services that may be of interest to our members.
